Supportify is a registered NDIS provider based in Pakenham, supporting people with disability — and their families — across South East Melbourne.
Who we are
We are a Pakenham-based team delivering everyday disability supports: help at home, personal care, getting out into the community, transport, and support coordination to make your NDIS plan work.
We keep the approach simple. We listen first, we explain your options in plain English, and we build support around what independence looks like for you — not around a template or our roster.
What that means in practice
- Consistent support workers wherever rosters allow, so you are not re-explaining yourself every week
- Same-gender support available on request for personal care
- Families, carers and support coordinators kept in the loop, with your consent
- Regular check-ins, and changes when your goals or needs change
Our NDIS registration
Registration means we are audited against the NDIS Practice Standards and must follow the NDIS Code of Conduct. We are approved for these registration groups:
